Two integration architectures
Interface-level permissions
Each interface holds its own credentials and scope. Policy is distributed across integrations and is difficult to review, change or revoke consistently.
Central determination layer
Every request — human or system — is evaluated against one policy layer that determines the permitted attribute subset, then the response is composed from that subset.
Standards such as FHIR describe how information may be exchanged. They do not decide whether a particular requester should receive a particular attribute for a particular purpose.
